Our referral process is simple and straightforward. Head over to our referral page to get started and connect them with the car they need.
Referrals can be made by Care Coordinators, Social Workers, Occupational Therapists, Resettlement Managers, Commissioners or Social Services for those people who do not receive support through a Care Plan Approach (CPA).
Eligibility Criteria
- Aged 18+
- Any gender identities
- A CPA report and recent risk assessment
- Service User is prepared to engage with support services provided
- Service User’s current ADL (Assessment of Independent Living Skills) identifies a degree of independence
- Commitment to address support needs.
- Has any of the following conditions/status
Mental health
Learning disability
Physical disability
Autism
ADHD
Forensic history
Probation
Addiction
- Drugs/alcohol
- Gambling
- Forensic history

Our goal is to support residents to progress and live an independent life within 2 years. However, we understand that some individuals may have complex needs and require more time to complete this transition, The length of stay is dependent on the progress of the individual.
